A considered look at daily life - not a quick checklist
During the assessment we look at how you move around your home, manage everyday tasks, and where confidence or safety has changed. We take the time to understand the whole picture, including what matters most to you.
Afterwards you receive a written report in plain English: prioritised, practical recommendations to act on in your own time - and share with family or other professionals if you wish.

What's included
Free 15-minute suitability call
A short conversation to make sure the Home Function Review is the right fit before anything is booked.
Remote video assessment
A structured video session with Mariam Daji, HCPC-registered Occupational Therapist. Family members are welcome to join.
Written report
A prioritised plan in plain English - what to address first, what can wait, and the practical next steps to take. Yours to keep and share with anyone helping you. Delivered within five working days.
Follow-up call
Time to talk through the report together and answer any questions you or your family have.
Evening appointments are available Monday to Friday, plus a Saturday daytime slot - designed around working families.

Loved ones can join from anywhere in the UK
Because the assessment is on video, a daughter in one city, a son in another and a paid carer in the home can all be on the same call as the person being assessed. Everyone hears the same conversation and the same plan, with no second-hand retelling.
If a GP, social worker or discharge nurse is involved, they are welcome to dial in too. Nothing is shared without your consent.
What a remote assessment actually looks like
Before the call
You share a few photos of the rooms that matter - stairs, bathroom, kitchen. We send simple guidance on what helps.
During the call
Mariam guides a gentle walk-through of the home on video, room by room, watching how everyday tasks really go. Nobody is asked to do anything that feels unsafe.
After the call
You receive a written plan setting out what to address first, what can wait, and the practical next steps to take - then a 30-minute call together to talk it through. Yours to keep and share with anyone helping you.
What the review does - and doesn't - cover
What's included
- Assessment of daily function and home safety
- Prioritised, written recommendations
- Practical strategies and adaptations to consider
- Guidance you can share with family or professionals
What it isn't
- Ongoing therapy or hands-on treatment
- An urgent or emergency service
- The sale or supply of equipment
- A replacement for in-person assessment where that is needed
Some situations are better served by an in-person assessment. If that applies to you, we will say so during the free suitability call.
It may help if you recognise any of these
Daily tasks are harder
Washing, dressing or preparing meals has become more difficult or tiring.
Confidence has dipped
Moving around the home feels less steady or more worrying than it did.
Falls or near-misses
There have been trips, stumbles or falls in recent weeks or months.
After a hospital stay
You're adjusting at home following discharge, illness or a change in health.
Not sure if it's right for your situation?
Request a free suitability call. If it isn't the right fit, we'll tell you - and point you in a better direction.
