The Home Function Review is a private, independent occupational therapy assessment delivered remotely across the UK. It gives the person, their family and involved professionals a clear, structured view of how daily life is being managed at home and what should happen next.

We work alongside GPs, social workers, discharge teams and care providers when a timely, independent view of function at home would help clarify the next step.

What's included

A complete, structured review

  1. Free suitability call

    We check that the service and remote format are appropriate before anything is booked or paid for. If they are not, we will say so and suggest more suitable routes.

  2. 90-minute remote video assessment

    A structured assessment with the person and, where helpful, family or carers, including observation of relevant parts of the home.

  3. Written report within five working days

    A prioritised, plain-English plan covering functional concerns, risks, practical recommendations and next steps. It can be shared with family and relevant professionals with the person's consent.

  4. 30-minute follow-up call

    Time for the person or family to discuss the report and clarify what to do next. An involved professional may join where helpful and with consent.

How to refer

Two ways to start

Send the client to us

Ask the client (or family) to submit an enquiry themselves and mention your name. We'll arrange the free suitability call directly.

2.

Email us with consent

Email us the client's name, contact details and a brief reason for referral. We'll only contact them once you confirm consent has been obtained.
